1、建筑業(yè)企業(yè)資質(zhì)證書變更申請審核表；

2、《營業(yè)執(zhí)照》正副本復(fù)印件；
2. Copy of original and duplicate of business license;
3、原企業(yè)《營業(yè)執(zhí)照》副本復(fù)印件；

4、原建筑業(yè)企業(yè)資質(zhì)證書原件及復(fù)印件；

5、企業(yè)章程復(fù)印件（含原企業(yè)和新企業(yè)）；

資質(zhì)轉(zhuǎn)讓流程
Qualification transfer process
1．企業(yè)在報送書面申請材料前，應(yīng)登錄省建筑業(yè)企業(yè)網(wǎng)上快報系統(tǒng)/企業(yè)資質(zhì)管理系統(tǒng)/資質(zhì)證書變更/填寫變更事項并確定變更，完成網(wǎng)上申請、數(shù)據(jù)上傳；
1. Before submitting the written application materials, the enterprise shall first log in to the provincial construction enterprise online express system / enterprise qualification management system / qualification certificate change / fill in the change items and determine the change, and complete the online application and data upload;
2．區(qū)縣建管部門審核簽字；
2. Reviewed and signed by the district and county construction management department;
3．市建管處審核；
3. Reviewed by the municipal construction management office;
4．省核準(zhǔn)企業(yè)的名稱變更到省建管局審核辦理；
